Public Library Information Update

Each fall (after new board members have been appointed and the chair has been elected) library managers complete all fields and submit the form. Throughout the year, submit this form to notify Parkland of any changes (e.g. contact information, library hours, staff changes).
Type "N/A" if library cannot send or receive faxes
Indicate fees for each type of card (e.g. adult, junior, family, non-resident, etc.)
Include any other information Parkland should know
